你叫什么名字 nǐ jiào shén me mínɡ zi  (what’s your name)

It is used to ask someone’s full name, but not those older than oneself or in a higher position.
Usually, both the surname and the first name are given in response.

     =  nǐ  ( you )
     =  jiào  ( call /name )
什么 =  shénme  ( what )
名字 =  mínɡzi  ( name )

Example :

A : 你叫什么名字?
nǐ jiào shénme mínɡzi
 (What’s your name)

B : 我叫 王吉才
wǒ jiào wánɡ jí cái
(My name is Wang ji cai)

NOTE
  我 =  wǒ  =  I , me  , my
   = jiào  ( call /name )

    (wang) is surnam
